Chambersburg Pharmacy Identifies Patient at High Risk of Falling

Keystone Pharmacy in Chambersburg identified a patient who had experienced three falls during the past year while conducting a routine medication synchronization review.

The pharmacy team is working with the patient to address factors contributing to the high risk of falling, including unstable housing.

The Pennsylvania Pharmacists Association shared the case during the National Council on Aging’s Falls Prevention Awareness Week. The council offers a 13-question online assessment that provides older adults with a personalized fall-risk score and guidance for reducing their risk.

Falls prevention resources are available at ncoa.org. The Falls Free CheckUp is also available online.
