Partnership to Enhance Behavioral Health Care Access

Concert Health has partnered with WellSpan Health to improve access to behavioral health services in primary care settings in South Central Pennsylvania and northern Maryland. This collaboration will integrate Concert Health’s collaborative care model with WellSpan’s existing healthcare services. Patients referred by WellSpan can expect contact within 24-48 hours for symptom monitoring, medication management, and counseling interventions.

The collaborative care approach, which is evidence-based, aims to treat behavioral health conditions like anxiety and depression directly within primary care. Concert Health’s strategy includes a mix of their behavioral health team and a technology platform that enhances communication between patients and their primary care providers.

WellSpan Health, an integrated delivery system, incorporates over 2,000 providers and numerous facilities, focusing on comprehensive, value-based care. Their existing services include a range of behavioral health treatments, from inpatient care to telemedicine.

Concert Health’s services have reached nearly 100,000 patients, showing significant outcomes in symptom reduction and patient satisfaction. Their collaborative care model is recognized for its efficiency in addressing the mental health crisis, complementing the traditional health services offered by WellSpan.

Ethel was a very loving and caring woman. She will always be remembered and cherished as a loving wife, mother, grandmother, and friend.

Sharon M. Matthews 1951-2024

Sharon enjoyed game shows on TV, mostly Wheel of Fortune. She also liked watching the Food Network and talk about what they made.

Ralph V. Foltz 1928-2024

Ralph enjoyed hunting, telling stories, and traveling around the United States on bus trips, alongside his wife Lois.
